The Pixel Watch app on the phone shows 'Watch is offline' when neither Bluetooth nor WiFi can reach the watch.
The watch itself keeps recording — data is not lost — but features that need the phone (notifications, app installs, sync) are paused.
Most cases clear by toggling Bluetooth, force-quitting the Pixel Watch app, and ensuring the watch is on the same WiFi as the phone.

How to Fix It

Check Bluetooth on the phone.
Settings → Bluetooth → on.

Wear the watch close to the phone.
Bluetooth Low Energy works best within a few feet.
If the watch is on a charger in the bedroom and your phone is in the kitchen, the link is unreliable.
Bring them within 10 feet. -
Confirm watch is on the same WiFi.
Watch Settings → Connectivity → WiFi.
The watch should show the same WiFi name your phone uses.
